Thalassemia Test: Name, Procedure and Price in Pakistan
A thalassemia test is a blood test that shows whether you carry the thalassemia gene. It takes one ordinary blood sample, needs no fasting and no preparation, and for most people the result settles the question for life — carrier status does not change.
What is the thalassemia test called?
There is no single test named “thalassemia test” on a lab menu, which is why people are often sent away confused. Two tests are involved, usually in this order:
- CBC (complete blood count) — sometimes written CP or blood CP in Pakistan. This is the screening step. Carriers characteristically show small, pale red cells: MCV around 60–70 fL and MCH around 19–23 pg, often with a red cell count that is normal or raised rather than low.
- Haemoglobin electrophoresis, or HPLC (high-performance liquid chromatography) — the confirmatory test. It measures the different types of haemoglobin present and reports HbA2. An HbA2 of roughly 4–8% indicates beta-thalassemia trait.
If you are asking a lab for the right thing, ask for Hb electrophoresis with HbA2, or say you want thalassemia carrier screening. Many labs in Pakistan also sell a bundled “thalassemia screening profile” that includes the CBC.
Why iron deficiency confuses this test
Iron deficiency anaemia and thalassemia trait both produce small, pale red cells, so a CBC alone cannot tell them apart — and iron deficiency is extremely common in Pakistan, particularly in women. Worse, iron deficiency can push HbA2 down and mask a carrier result.
This is why doctors usually check iron status (ferritin or transferrin saturation) alongside, and may want iron deficiency corrected before they trust a borderline HbA2. If you were told years ago that you were “just anaemic” and given iron tablets that did not help much, that is a reason to ask specifically about thalassemia trait.
What about RDW?
RDW (red cell distribution width) tends to be normal in thalassemia trait and raised in iron deficiency, and indices such as the Mentzer index use the relationship between MCV and red cell count in the same way. These are useful pointers, but they overlap too much to be conclusive on their own. Only HbA2 on electrophoresis or HPLC confirms the diagnosis.

Haemoglobin electrophoresis is the test that carries the cost. As of September 2026, listed prices at major private labs were roughly Rs 2,000 to Rs 3,800, with individual labs including Chughtai Lab, Islamabad Diagnostic Centre and Al-Nasar Lab quoting inside that band. A CBC on its own is far cheaper, usually a few hundred rupees.
Two things worth knowing before you pay private rates:
- Thalassemia foundations often screen free or at token cost. Fatimid, Sundas, Alkhidmat and similar centres run screening as part of their prevention work. For a straightforward premarital check this is usually the cheapest route.
- Government hospitals and provincial prevention programmes also provide screening, and where premarital screening is legally required the associated testing is generally arranged through designated facilities.

- Anyone marrying a cousin or within an extended family that has any history of the condition.
- Anyone with a persistent mild anaemia that iron tablets never fixed.
- Couples already expecting, if neither has been screened — prenatal diagnosis is possible and time-sensitive.
What the result means
- Normal HbA2, normal indices — you are not a beta-thalassemia carrier.
- HbA2 raised to about 4–8% — beta-thalassemia trait. You are healthy and need no treatment. What matters now is whether your partner is also a carrier.
- Both partners carriers — ask for genetic counselling. Every pregnancy carries a 25% chance of thalassemia major, and prenatal diagnosis is available.
- Borderline or confusing result — usually iron deficiency interfering, or occasionally alpha-thalassemia, which electrophoresis does not reliably detect and which may need DNA testing.
A result is a medical document, not a verdict to interpret alone. Take it to a doctor, particularly if both partners test positive.
